no you can't be unbanned if you got perma for flame cheats etc
they only unbann accoutns that had some problems with payment for rp etc etc

if you didnt play from the same ip when you got banned.
you can tell them you got hacked if you write the ticket well your account will get recovered.

first i read this in other forums it worked for some people who replied there.
second there is a chance to get unbanned if you flame as some people in elitepvpers and other forums confirmed it ( you have just to see the unbanning service threads there is people who confirmed it worked for them).
riot themselves said that some case of perma ban can be reviewed and its possible to unban if they accused them wrongly for exmple getting hacked( when you get hacked riot can even give you back the rp spent by the hacker)

I manage to unban my acc back in 2014 but it was very hard to convince them. But there is always chance try it.
